Patients increasingly ask whether they can come off their diabetes medication altogether, and for some the answer is yes. In DiRECT, 46% of intervention participants were in remission at 12 months, and remission rose with the weight lost. But remission is not cure, relapse is common over time, and a foot with a history of diabetes does not reset its risk because HbA1c has normalised.
The module defines remission by the three-part 2021 consensus and sets out a 2025 critique of it. It explains the twin cycle hypothesis and why remission is more achievable earlier, compares relapse after lifestyle intervention, gastric bypass and sleeve gastrectomy, and gives a candidate profile and referral route. For the foot, it teaches that surveillance continues on its own merits, that rapid weight loss can change foot shape and pressure, and that a painful treatment-induced neuropathy can occasionally follow rapid glycaemic improvement.
